Eligibility
Procreate Foundation Grant
If you plan to enter an IVF cycle to increase the chances of having a child and you feel you might need financial assistance, you may be eligible for a Procreate Foundation grant. Eligibility Requirements:
- Diagnosis of infertility
- Under the care of a reproductive endocrinologist
- Previously applied for funding through a financial institution and received a denial
- U.S. citizen or permanent resident
Application
A concrete help for a better world
To apply for a Procreate Foundation grant, please complete our application and include the following supplementary information:
- Fertility Journey Essay (2 pages, or no more than 1,000 words)
- Tax documents for income verification
- Letter from treating provider (fertility specialist) - Diagnosis - Protocol/Treatment Plan - Itemized cost of treatment
- Please include a $50.00 application fee with your submission of your application. (Note this is non-refundable)
