Purpose of the Site
The Site provides general information about the Foundation’s financial assistance grants, eligibility requirements, ways to donate and how to contact us. The Site does not provide medical advice. Decisions about fertility treatment should be made with your own physician.
Grant applications
Information on the Site about eligibility and grant amounts is general. Submitting an application does not guarantee an award. Grants are decided by the Foundation at its discretion, subject to available funds. The application fee is non-refundable. Applications are not submitted through this Site; see the Applicants page for how to apply.
Donations
Donations are voluntary and non-refundable unless required by law. Payments are processed by TransactionExpress (TSYS) or PayPal on their own secure pages and are subject to their terms. Procreate Foundation is a 501(c)(3) organization (Tax ID 84-4318313); donations may be tax-deductible to the extent permitted by law. Consult your tax advisor.
Use of the contact form
The contact form is for general inquiries only. Do not include medical, diagnostic or other health information in it. By submitting the form you accept our Privacy Policy and these Terms.
Intellectual property
All content on the Site (text, logos, images, design) belongs to Procreate Foundation or its licensors and may not be copied, reproduced or distributed without written permission, except for personal, non-commercial viewing.
Prohibited conduct
You may not use the Site to send unsolicited messages, attempt to access restricted areas, interfere with its operation, scrape its content or submit false information.
Third-party links
The Site contains links to third-party websites (payment providers, social media, affiliated organizations). We are not responsible for their content or privacy practices.
Disclaimer and limitation of liability
The Site is provided “as is” without warranties of any kind. To the fullest extent permitted by law, Procreate Foundation is not liable for any damages arising from the use of, or inability to use, the Site.
Governing law
These Terms are governed by the laws of the Commonwealth of Virginia. Any dispute will be resolved in the state or federal courts located in Virginia.
